Just go to the work page, click "Add a file to this work page", and then "Use this form for files already uploaded" under the File Name box on the submission form.

Hi there!

So, if you go to the work page (that you listed) you'll see the 'Add a file to this work page' link. Click on it, and then you'll see a link, right below the 'file name' field, that says, 'use this form for files already uploaded', which will take you to the old submission form where you can specify an already-uploaded file.

Two notes, though: in the future, you can just use the initial form that's linked to from the work page, and upload the file directly (the new form takes care of everything at once!). Also, please indicate the composer in the name of the file.
